Example pack: fictional person
Mabel, Quillmere House and every name of staff and family in this example are invented.
What the pack is
Prepare turns the care records you have into a pack for the assessment meeting. Every point in it shows the page it came from, with a short quote, so you can find it quickly if anyone asks.
It also shows where the evidence is thin, the records that usually help, and a request you can send. And it gives questions you could ask at the meeting. There is a section for the family's view that you fill in yourself.
It does not say whether the person will get NHS Continuing Healthcare. Only the NHS decides that.
Each quote in the pack is checked word for word against the record. The short summaries are written by software, which also checks them, but they can still be wrong, so the quote is always shown next to them.
How this example was made
This example was made from typed records. Prepare reads typed records only for now. It can't read handwriting, or scans and photos, yet.
This example is shown exactly as the software made it. Nothing was removed or changed by hand. Your pack goes through the same steps and checks as this example. Before we published this example, we also checked each summary against its quote once more. Your pack doesn't get that extra check, and it may contain mistakes. That is why every point shows its quote.
In this example each record has a name and a date, because the invented records were labelled when they were made. In your pack, each source is shown as a record and its page number, with the date when the page gives one.
The fictional person
Mabel is 86 and has lived in a nursing home since late August, after a hospital stay for a chest infection and a fall. She has vascular dementia and a lung condition called COPD, and staff help her to move, eat, wash and take her medicines. Her pack below was made from 12 pages of her invented care records, including daily notes, a hospital letter and her medicines chart.
