How to use this pack

  1. This pack brings together what the care records you gave us say about Mabel's care needs. It is set out under the 12 areas of need that the assessment team looks at.
  2. Every point shows where it came from: the record, the page and the date. There is also a short quote, so you can check it and find it quickly.
  3. Read the evidence for each area. If something is missing or wrong, say so at the meeting.
  4. Use the requests to ask for any records that are missing. Send them as soon as you can.
  5. Take the questions with you. You don't have to ask them all.
  6. Fill in the family's view in your own words. You know Mabel in ways the records don't. Start with the areas you know best.
  7. This pack is not legal or clinical advice. The assessment team and the ICB make the decision.

The family's view

In our words

Written by the family. Nobody has written it for you. Leaving boxes blank is fine.

What does a typical day look like for Mabel now?

What happens at night?

What has changed in the last three months?

What do staff do for Mabel that Mabel could once do alone?

Is there anything the records don't show?
