Terms of service

These terms are for Prepare and your CHC Ready account. If you paid before Prepare opened, the terms you agreed to then still apply to you as well. Email hello@chcready.co.uk for a copy.

1. Who we are. CHC Ready is a service run by Dispono Ltd, registered in England and Wales, company number 17529136, registered office Apartment 47, The Pinnacle, 8 School Road, Hove, BN3 5EU. Contact: hello@chcready.co.uk.

2. What Prepare does. You upload typed care records you hold, up to about 300 pages. We organise what they say about the person's needs under the 12 areas of the Decision Support Tool, each point with the record and page it came from. You also get a list of records that seem to be missing and questions for the meeting. Prepare does not predict or decide eligibility; the ICB decides. It is not legal or clinical advice.

3. Your authority. You may only upload records if you have a health and welfare lasting power of attorney or a deputyship, the person agrees, or you are a close relative or friend involved in their care and acting in their interests. You must have obtained the records lawfully. Uploading records without this right is a breach of these terms, and we will delete them and close the account.

4. Accuracy. We check every point against its quote and page, but the records and the program can be wrong. Read the pack before you use it, and mark anything that looks wrong. Always rely on the original records.

5. Price and payment. £79, one payment, taken by Stripe before your records are processed.

6. Refunds. You can cancel for a full refund at any time before we start processing your records. After your pack is ready, you have 14 days to ask for a full refund, for any reason. This is more than the law requires. Nothing here affects your legal rights.

7. Your data. Records and your pack are deleted 30 days after the pack is ready, or sooner if you ask. See the privacy notice.

8. Our responsibility. We provide the service with reasonable care and skill. We are not responsible for the outcome of any assessment. Nothing in these terms limits liability that cannot be limited by law.

9. Complaints. Email hello@chcready.co.uk. We reply within 5 working days.

These terms are governed by the law of England and Wales.
