Fast Track

If the person's health is getting worse quickly and they may be near the end of life, talk to their doctor or nurse today about the Fast Track. You do not need this website for that.

What it is

Fast Track is a quicker route into NHS Continuing Healthcare. It is for people whose condition is getting worse quickly and may be entering a terminal phase.

It skips the checklist and the full assessment. When the form is filled in properly, the ICB must act on it straight away.

Who fills in the form

A doctor or registered nurse who is involved in the person's care fills it in. This can include hospice staff.

Anyone else supporting the person can ask that doctor or nurse to consider it.

Things worth knowing

  • It can be used at home, in a care home, or anywhere else.
  • It is not only for people expected to die within days.
  • The care may be reviewed later, so funding could change.

Who to talk to today

  • The person's GP, hospital doctor, ward nurse, district nurse or hospice team.
  • For free, independent advice, call Beacon on 0345 548 0300.
